Technical Assistance:
The Licensing Inspector has reviewed with the Provider COVID-19 Essential Guidance for Child Care programs.

Further discussion included: The renewal process - A blank page 1 of the FDH Renewal Application was sent to be completed correctly (without a fictitious name). A blank Business Entity - Sole Proprietor page was also sent to be completed. A blank EP&RP was requested by the Provider to be sent in order to update the one he originally completed.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A renewal inspection was initiated and concluded on 05/28/2021. The Provider was contacted by telephone to initiate the inspection. There were (3) children present, (8) points, with (2) caregivers. The inspector emailed the Provider a list of items required to complete the inspection. The Inspector reviewed (2) children?s records, (1) staff record, and (1) household member record submitted by the Family Day Home (FDH) to ensure documentation was complete.

Information gathered during the inspection determined (2) non-compliances with applicable standards or law and were documented on a Violation Notice issued to the FDH.

If you should have any questions, please call or send an email to charles.perkins@dss.virginia.gov. Thank you.
