Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on May 27, 2021 and concluded on June 1, 2021. The provider was contacted by telephone to initiate the inspection. There was one child present and one caregiver. The inspector emailed the provider a list of items required to complete the inspection. The inspector reviewed two children?s records, one caregiver record and one household hold member record submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care Programs.

Description:	Based on review, provider did not ensure caregiver and adult household member have an updated TB test/screening every 2 years. Evidence: Provider's most recent TB test was dated 4/5/2018 and adult household member's most recent TB test was dated 4/19/2018.
Plan of Correction:	Provider and household member will have a TB test or screening completed.

Description:	Based on review, provider did not ensure that caregiver and household members 14 and older have an updated central registry child abuse and neglect search every 5 years. Evidence: Provider and household member have a central registry child abuse and neglect search dated 1/29/2016 which is more than 5 years old.
Plan of Correction:	New central registry checks were just filled out and submitted to the central registry unit.
