All child care programs must continue measures to keep children physically distanced and with consistent caregivers, use face coverings, monitor children and staff for symptoms, and maintain ongoing compliance with the Virginia Department of Health (VDH) and Centers for Disease Control and Prevention (CDC) health and safety recommendations. All programs are required to notify VDH and the licensing inspector and follow all recommendations if there is a confirmation of a positive case of COVID-19.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was conducted on 6/4/2021 with an exit interview on 6/7/2021. The provider was contacted by telephone and a virtual inspection was conducted. There were six children present, ranging from three years to ten years of age, for a total of seven points. The Inspector reviewed two children's records, two caregiver/household member records, the emergency drill log, hand washing procedures, enhanced cleaning and sanitizing of toys and surfaces, field trips, and daily indoor and outdoor activities. The information gathered during the inspection determined non-compliance with standards that are documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-111-60-B
Description: Based on review of two children's records, it was determined that not all records contained all required information. Evidence: the records for child #1 and child #2 did not have the name, address or phone number for a second emergency contact or the policy number for the child's medical insurance.

Plan of Correction: Will obtain the missing information.

Standard #: 22VAC40-111-180-A
Description: Based on review of two caregiver/household member records, not all caregivers/household members had a repeat tuberculosis (TB) test/screening at least every two years from the date of the initial screening or testing. Evidence: the TB test/screening for caregiver/household member #1 is dated: 2/26/2019. the TB test/screening for caregiver/household member #2 is dated 2/25/2019.

Plan of Correction: The provider stated that they will make appointments to get an updated TB test/screening this week.
