Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A subsidy health and safety inspection was conducted on 06/04/2021 and 06/08/2021. The provider was contacted by telephone to initiate the inspection. There were 8 children present with 2 caregivers. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection.

The information gathered during the inspection determined non-compliances with subsidy regulations, and violations were documented on the violation notice issued to the vendor.

Violations:
Standard #: 22VAC40-665-160-B
Description: Based on record review and interview, the vendor failed to maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs.

Evidence:
1. The written attendance sheet for the family day home did not list the arrival and departure time of children.
2. The vendor stated that the arrival and departure times were not listed on the attendance sheet because she was not aware of this requirement.

Plan of Correction: Arrival and departure times will be noted on the attendance sheet going forward.

Standard #: 22VAC40-665-230-B
Description: Based on interview, the vendor failed to ensure that within the first 90 days of employment or service all caregivers shall complete Virginia Preservice Training for Child Care Staff sponsored by the Department of Social Services.

Evidence:
1. The provider stated that caregiver 2 had not completed the ten hour Virginia Preservice Training.
a. Caregiver 2 has an employment date of 12/12/2017.

Plan of Correction: Caregiver 2 will complete this training.

Standard #: 22VAC40-665-230-H
Description: Based on interview, the vendor failed to ensure that caregivers who work directly with children shall, in addition to preservice and orientation training required in subsections A through D of this section, annually attend at least 16 hours of training, to include the department's health and safety update course.

Evidence:
The provider stated that caregiver 1 and caregiver 2 had not completed the three hour health and safety updated course.

Plan of Correction: Both caregivers will complete this 3 hour on-line training.

Standard #: 22VAC40-665-410-A-2
Description: Based on record review, the vendor failed to ensure that shelter-in-place procedures shall be practiced twice a year.

Evidence:
1. The emergency practice log for the family day home indicated that only one shelter-in-place practice drill had been conducted in 2020.
a. Two shelter-in-place practice drills are required annually.

Plan of Correction: Two shelter-in-place practice drills will be conducted in 2021.
