Technical Assistance:
Discussed the following:
a. Standard 800.A-3 - Provisions for a responsible person who is 18 years of age or older and is able to arrive at the family day home within 10 minutes for emergency backup care until the children can be picked up by their parents.

Comments:
This inspection was conducted by licensing staff using an alternate protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on 06/04/2021 and concluded on 06/08/2021. The provider was contacted by telephone to initiate the inspection. There were 8 children present with 2 caregivers. The inspector emailed the provider a list of items required to complete the inspection. The inspector reviewed 2 staff records and 4 children's records submitted by the provider to ensure documentation was complete.

Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility.

The licensing inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care Programs.

Violations:
Standard #: 22VAC40-111-90-C
Description: Based on record review, the provider failed to obtain documentation of additional immunizations for a child once every six months for children under the age of two years.

Evidence:
1. The most recent immunizations for child 1 were administered on 12/11/2019 according to the written immunization record on file.
a. Child 1 is under the age of 2 years.

Plan of Correction: An updated immunization record will be requested from the parent.

Standard #: 22VAC40-111-170-A
Description: Based on record review, the provider failed to obtain from each adult household member prior to coming into contact with children a current Report of Tuberculosis Screening form published by the Virginia Department of Health or a form consistent with it documenting the absence of tuberculosis in a communicable form.

Evidence:
1. The tuberculosis screening for household member 1 was obtained after he became a resident of the home.
a. Household member 1 has resided in the home since May 5, 2020 however, a tuberculosis screening was not obtained until June 8, 2021.

Plan of Correction: All adult household members will obtain a TB screening when they become a resident of the home.

Standard #: 22VAC40-111-210-A
Description: Based on interview, the provider failed to ensure that all caregivers shall obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.

Evidence:
The provider stated that no training classes had been obtained by caregiver 1 and caregiver 2 in 2020 or 2021.

Plan of Correction: Caregivers will begin taking training classes in order to complete 16 hours of training by the end of 2021.
