Technical Assistance:
1. Discussed health + safety practices.
2. Outdoor play equipment, such as a slide, should not be placed on any hard surface, such as brick. The provider reported she has a mat that is placed under the slide, but the inspector was not able to observe and assess compliance today. Any mat used should cover the entire slide area with a 6' circumference, and have a thick resilient padding.
3. The license will not be issued until the background check non-compliances are completed. CORRECTED 6/23/21

Comments:
This inspection was conducted by staff using an alternative remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. An on-site inspection of the home was not conducted at this time however, a portion of the inspection included a virtual component which allowed the inspector to observe the daycare areas of the home.

A scheduled initial family day home inspection was conducted at 10:00am. The provider was contacted by phone to initiate the inspection, and given a list of items required to complete the inspection. There were 3 children present = 7 points with the provider + Adult #1. Ratios and supervision were in compliance. Children were playing with toys inside the home at the time of inspection. A daily schedule is in place, and includes indoor and outdoor activities. The daycare areas are located on the lower level of the home and were well supplied with toys + equipment for the children. Areas appeared to be clean and organized today. The outdoor play area is on a brick patio covered with artificial grass at the rear of the home. Information gathered during the inspection determined there were areas of non-compliance with applicable standards or laws reviewed. Description: Based on records reviewed, it was determined that 1 of 1 child records did not include a physical exam in the first 30-days of attendance. Evidence: Child #1 - First date of attendance was 5/17/21, and there was no documentation of a physical exam.

Plan of Correction: Physical exams will be in each child's record within the first 30-days of enrollment.

Standard #: 22VAC40-111-60-B
Description: Based on records reviewed, it was determined that 1 of 1 child records did not include required information. Evidence: Child #1 - There were no addresses for the emergency contacts listed.

Plan of Correction: Addresses for all individuals listed as an emergency contact will be written in each child's record.

Standard #: 63.2-1721.1-B-4
Description: Based on records reviewed, it was determined that 1 of 4 household members did not have required out-of-state background checks. Evidence: Adult #4 - Did not have a completed out-of-state criminal record check, or sex offender registry check.

Plan of Correction: The out-of-state background checks will be completed promptly.
