Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review and a virtual tour of the program.

A monitoring inspection was initiated on and concluded on 7/26/2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 3 children present, ranging in ages from 3 years old to 7 years old, with 1 caregiver supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 4 caregiver/household records were reviewed.

Violations:
Standard #: 22VAC40-111-170-A
Description: Based on interview, the provider did not obtain a tuberculosis (TB) test or screening for each adult household member.
Evidence:
1. Adult Household member #3 became an adult on 6/11/2021 and the provider did not have the TB test or screening.

Plan of Correction: I will schedule an appointment today.

Standard #: 22VAC40-111-180-A
Description: Based on interview, the provider did not obtain updated tuberculosis (TB) tests or screenings for each household member every two years.
Evidence:
1. The provider stated Household #2 had not received an updated TB test or screening. Last date of test or screening was on 8/17/2018.

Plan of Correction: I will schedule an appointment today.

Standard #: 22VAC40-111-210-A
Description: Based on review of training, the caregiver did not obtain a minimum of sixteen clock hours of training annually.
Evidence:
1. The provider had a total of 9 hours of training in 2020.

Plan of Correction: I will complete the 7 hours of training and make sure I complete 16 hours each year.

Standard #: 22VAC40-191-40-D-4-a
Description: Based on interview, the provider did not obtain the sworn statement within 30 days of an adult residing in their home.
Evidence:
1. Household #3 became an adult in June 2021 and the provider did not obtain the sworn statement within 30 days.

Plan of Correction: I will ask her to do it.

Description: Based on interview, the provider did not obtain the fingerprint results once a household member became an adult.
