This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contact, documentation review, interview, and a virtual tour of the program.
A renewal inspection was initiated on 7/26/21 and concluded on 7/28/21. The provider was contacted by telephone and a virtual inspection was conducted. There were no children present with the provider today, though three children are enrolled. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies, and nutrition. A total of two child records, one staff record, and five household member records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22VAC40-111-60-B Description: Based on review of two children's records, the provider did not obtain the full address of two designated persons to contact in case of an emergency if the parent cannot be reached. Evidence: The records for Child A and Child B did not contain the house number or street name of either emergency contact. Plan of Correction: I will be sure to get this information from the parent tomorrow.
Standard #: 22VAC40-111-180-A Description: Based on documentation review, the provider did not obtain subsequent tuberculosis (TB) screening or testing at least every two years from the date of the initial screening or testing for staff/household members. Evidence: The TB tests on record for Staff 1 and Household Members A and D were dated 3/5/19; the TB tests on record for Household Members B and C were dated 2/28/19. Plan of Correction: I will make sure the TB tests are completed as soon as possible.
