A subsidy health and safety inspection was initiated on July 28, 2021 and concluded on July 29, 2021. The provider was contacted by telephone to initiate the inspection. There were seven children present and one staff. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection.
The information gathered during the inspection determined non-compliance(s) with subsidy regulations, and violations were documented on the violation notice issued to the facility.

Standard #: 22VAC40-665-230-H Description: Based on review of records and interview, the vendor did not ensure all caregivers complete the department's health and safety update course.
Evidence:
The record for caregiver #1, did not have documentation of completing the annual health and safety training within the past 12 months.Plan of Correction: Will take training as soon as possible.
