Standard #: 22VAC40-111-100-A Description: Based on children's records, 2 out of 2 records, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence: Child #1 (date of enrollment 6/08/2021) did not have a physical examination in file to review during the inspection.Plan of Correction: Mom sent the physical but it was missing the doctors signature, so mom resent a new form with the docotrs signature and was placed in her file.
