Technical Assistance:
All caregivers must obtain 16 hours of training annually.

Effective September 1, 2021 all child care facilities must have a Carbon Monoxide Detector.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A monitoring inspection was initiated and concluded on August 5, 2021. The provider was contacted by telephone and a virtual inspection was conducted. There was 1 child present, aged 10 month with 1 caregiver. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 2 caregiver records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-111-60-A
Description: Based on a review of children's records, the provider failed to ensure that all required documents are obtained for each child enrolled.

Evidence:
Child #1, #2 and #3's records do not have documentation of the name, address and phone number of two designated person to contact in case of an emergency it the parent can not be reached.

Plan of Correction: I will get the emergency contacts from parents and add them to children's records.

Standard #: 22VAC40-111-90-A
Description: Based on a review of children's records, the provider failed to obtain documentation that the child has been adequately immunized by the first day of attendance.

Evidence:
Child #3's records did not have documentation of immunization by the first day of attendance.
First day of attendance 04/21/2021 documentation of immunizations 05/12/2021.

Plan of Correction: I will make sure all newly enrolled children have immunization on the first they attend.

Standard #: 22VAC40-111-200-A
Description: Based on a review of care giver records, the provider failed to orient the assistant by the end of their first week of assuming job responsibilities.

Evidence:
Caregiver #2's record does not have documentation of orientation.

Plan of Correction: I will assure that all current and any new assistants have orientation training before the end of their first week of employment.
