This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A monitoring inspection was initiated and concluded on August 4, 2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 8 children present with 3 caregivers. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 3 caregiver records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22VAC40-111-100-A Description: Based on a review of records, the provider failed to obtain documentation of a physical examination by the 30th day of attendance.
Evidence:
Child #1's records did not contain documentation of a physical examination by the 30th day of attendance. The Child's first day of attendance was 4/5/2021.Plan of Correction: i will ensure that all children enrolled will have documentation of a physical by the 30th day of attendance.
Standard #: 22VAC40-111-90-A Description: Based on a review of children's records, the provider failed to obtain documentation that the child has been adequately immunized by the first day of attendance,
Evidence:
Child #1's record did not contain documentation of immunizations on the first day
Child #1's first day of attendance was 4/5/2021.Plan of Correction: I will make sure all newly enrolled children have documentation of immunizations on their first day of attendance,
Standard #: 22VAC40-191-60-C-2 Description: Based on a review of records, the provider failed to obtain a central registry finding within 30 days of employment.
Evidence:
Caregiver #3's records do not contain documentation of a central registry finding within 30 days of employment. First day of employment 6/10/20201.Plan of Correction: I will contact OBI to find out where the central registry check is in the process.
