This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and a virtual tour of the program.
A monitoring inspection was initiated and concluded on 8/3/2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 3 children present totaling 5 points, ranging in ages from 3 to 5, with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 1 child's record and 2 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22VAC40-111-60-B Description: Based on a review of children's records, it was determined that one record does not contain all required information.
Evidence:
Child record A does not include the last name for each emergency contact.Plan of Correction: I will ask the parent to update the information.
Standard #: 22VAC40-111-180-A Description: Based on a review of caregiver records, it was determined that the provider did not obtain for each caregiver a current Report of Tuberculosis Screening every two years from the date of the first screening.
Evidence:
The TB Screening for assistant caregiver A expired on 10/17/2020.Plan of Correction: I will remind her to get a new screening completed.
