This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A renewal inspection was initiated on 08/09/2021 and concluded on 08/12/2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 7 children present, ranging in ages from 3 years to 5 years, with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 2 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22VAC40-111-60-B Description: Based on record review, children's records did not contain all required information.
Evidence:
1) The record for Child #2 did not contain address and telephone number a second custodial parent's place of employment.Plan of Correction: I will contact the parent to provide the information.
Standard #: 22VAC40-111-180-A Description: Based on record review, the provider did not obtain for each caregiver current Report of Tuberculosis (Tb) Screening form, every two years from the date of the first screening.
Evidence:
1) The most recent Tb screening report in the record for Staff #1 was dated 08/08/2019. The most recent Tb screening report in the record for Staff #2 was dated 08/09/2019.Plan of Correction: Staff #1 and Staff #2 shall obtain current Tb screening reports.
