Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A monitoring inspection was initiated and concluded on August 17, 2021, There was one child present, with 1 caregiver. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervisions, programming, medication, special care and emergencies and nutrition.
A total of 2 children's records and 1 caregiver record were reviewed. The information gathered during the inspection determined violations which are included on the violation notice.

Violations:
Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records, the provider failed to obtain documentation of parents' place of employment, work address and work phone.

Evidence:
Child # 2's records do not have documentation of parent's work, work address or work phone number.

Plan of Correction: I will contact parent require that she complete the required information for children enrolled.

Standard #: 22VAC40-111-90-A
Description: Based on a review of children's records, the provider failed to obtain documentation of child's immunizations by the first day of attendance.

Evidence:
Child #1's records do not include documentation of receipt of immunizations on the first day of attendance. Child's first day of attendance was 5/21/2021 and immunizations are dated 5/27/2021.

Plan of Correction: I will make sure that all students enrolled will have proof of immunizations on the first day of attendance.
