54.1 Provider must be MAT certified to administer prescription medication.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review and a virtual tour of the program.

A renewal inspection was initiated on 8/13/2021 and concluded on 8/13/2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 6 children present, ranging in ages from 10 months to 6 years, with 1 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 1 staff record were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-111-830-B
Description: Based on record review, shelter-in-place procedures were not practiced a minimum of twice per year.

Evidence:
Only one shelter-in-place drill was documented in 2020.

Plan of Correction: Will make sure to do two shelter in place yearly.

Standard #: 22VAC40-111-100-A
Description: Based on record review, the provider did not obtain documentation of a physical examination by or under the direction of a physician within 30 days of attendance for a child in care.

Evidence:
Child#2's (FDA: 11/17/2014) record does not contain documentation of the child's physical examination.

Plan of Correction: Parent will bring in physical by the end of the month.

Standard #: 22VAC40-111-90-C
Description: Based on record review, the family day home did not obtain documentation of additional immunizations once between each child's fourth and sixth birthdays.

Evidence:
Child#2's record contains an immunization record that was last updated on 3/19/2016.

Plan of Correction: Parent will bring updated immunization records by the end of the month.
