Technical Assistance:
Ensuring the forms are updated and parent notified when policies or procedures are changed (i.e. the decision regarding the administration of medication) was discussed with the provider.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interview and a virtual tour of the program.
A monitoring inspection was initiated on 08/06/2021 and concluded on 08/18/2021. The provider was contacted by telephone to initiate the inspection. There were 6 children present, 13 points, and one caregiver. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 1 caregiver record, along with any requested program records submitted by the provider to determine if required documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-111-70-A
Description: Based on review of two children's records, the provider has not ensured that before a child's first day of attendance, the parents are provided in writing the providers decision regarding the administration of medication.
Evidence:
The provider was unable to show documentation that the parent of child 1 was provided, in writing, the provider's decision regarding the administration of medication.

Plan of Correction: The provider responded with the following:
Each parent will be required to to sign the Decision Regarding the Administration of Medication form. This form will be updated in all children's records to indicate that no medication (prescription, topical or over-the-counter) will be administered to children in care.

Standard #: 22VAC40-111-180-A
Description: Based upon review of one caregiver record and provider interview, the provider has not ensured that a repeat tuberculosis screening is obtained for each caregiver every two years from the date of the first screening or more frequently as recommended by a physician or the local health department.
Evidence:
1.The most recent tuberculosis screening in the record of caregiver 1 is dated 04/26/2018 which is not within the past two years.
2. The provider verified that documentation of a more recent tuberculosis screening has not been obtained.

Plan of Correction: The provider responded with the following:
A repeat tuberculosis screening will be obtained.
