Technical Assistance:
1. The Provider's Medication Administration Training (MAT) certification expired on 6/21/21. The State granted leniency for certain training/certifications during the Governor's State of Emergency, which ended 7/1/21. There is currently a 90-day grace period for updating MAT, if the provider accepts prescription medication to children in care. The due date is 10/1/21.

Comments:
This inspection was conducted by staff using an alternative remote protocol, including phone interview, records review, and a virtual observations. An on-site inspection was not conducted at this time.

An unannounced renewal inspection was conducted at 11:10am. The provider was contacted by phone to initiate the inspection, and given a list of items required to complete the inspection. There were 12 children present = 36 points with the provider, assistant #1 and #2. Ratios and supervision were in compliance at the time of the inspection. The home was well supplied with toys and equipment for children, and appeared clean and organized today. Information gathered during the inspection determined there were areas of non-compliance with applicable standards or laws reviewed. Standard #: 22VAC40-111-210-A
Description: Based on records reviewed, caregivers did not receive a minimum of 16 hours of annual training for the period of 8/2/20-8/1/21

Evidence:

1. Provider - 2 of 16 hours
2. Assistant #1 - 2.5 of 16 hours

Plan of Correction: Caregivers will participate in at least 16 hours of training every year.

Standard #: 63.2-1721.1-B-4
Description: Based on records reviewed, it was determined a household member does not have completed out-of-state background checks.

Evidence:

1. Household member #3 does not have a completed out-of-state sex offender registry check, and a completed out-of-state child abuse and neglect search.

Plan of Correction: The sex offender check will be completed immediately and the results sent to the inspector for review.

A request for the child abuse and neglect search will be requested within 10-days, and a copy of the request will be sent to the inspector for review.
