Technical Assistance:
Discussion of vendor status; according to the DOE website Ms. Hossain is currently listed as not being a subsidy vendor. However, prior to becoming licensed she was a participant within this program and is under the belief that this still applies to her. The Provider was encouraged to contact the vendor manager to inquire more information on this discrepancy and what she would need to do to change this status to being an active subsidy member.

Comments:
A renewal inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and an interview. Due to technical issues, a virtual tour of the program could not be administered.

This inspection was initiated and concluded on 09/03/2021. The Provider was contacted by telephone and an inspection was conducted. There were 2 children present (within the day care - 4 points) and 2 children in residence (1 point) (for an overall total of 5 points), ranging in preschool age, with 1 caregiver supervising. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; medication (no medication on site); special care and emergencies (all drills up to date; and nutrition (discussion of menu and posting). A total of 2 children records, 1 staff record, and 3 household records were reviewed.

Information gathered during the inspection determined that there was a non-compliance with an applicable standard. This violation was documented on a Violation Notice issued to the program.

Standard #: 22VAC40-111-780-B
Description: Based on review, 1 of 2 children files indicated that annually, the Provider did not review with the parent the emergency contact information (required in 8 VAC 20-780-60.B.2) to ensure the information is correct, and obtain the parent's signed acknowledgment of the review.

Evidence -
1. On the date of inspection (09/03/2021), the most recent documented review of the emergency contact information for Child #1 was signed and dated on 02/17/2020.
2. This was confirmed on the date of inspection by the Provider.

Plan of Correction: This review shall be updated, signed, and dated for this year. This process shall be maintained annually for all children in care.
