Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A monitoring inspection was initiated on 8/25/2021 and concluded on 9/2/2021. The Provider was contacted by telephone and a virtual inspection was conducted. There were 2 children present (6 pts), ranging in ages from 3 months to 2 years, with 1 caregiver supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records, 1 caregiver records, and 2 household member records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. If you have any questions, please contact me at shahana.green@doe.virginia.gov.

Violations:
Standard #: 22VAC40-111-180-A
Description: Based on a review of household member records, 2 out of 2 household members did not have subsequent Tuberculosis screening every two years from the date of the first screening.

Evidence:
1. Household member #1's most recent TB test was dated 4/2/2019.
2. Household member #2's most recent TB test was dated 6/13/2019.

Plan of Correction: They will get their updated TB screenings as soon as possible.
