Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews (if applicable) and a virtual tour of the program (if applicable). A monitoring inspection was initiated on September 8, 2021 and concluded on September 9, 2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 6 children present, ranging in ages from 3 months to 5 years, with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 2 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #:	22VAC40-111-100-B
Description:	Based on review of two children's records, one does not contain the required documentation. Evidence: The record for child #1 does not contain documentation of a physical examination prior to first day of attendance.
Plan of Correction:	I will get a copy of their physical and make sure to have one before a child's first day.
Standard #:	22VAC40-111-180-A
Description:	Based on review of two household member records, one does not contain the required documentation of a current tuberculosis (TB) screening. Evidence: The record for household member #1 does not contain documentation of a current TB screening. Current documentation expired in March 2021.
Plan of Correction:	A new TB screening will be completed.
