Comments:
The inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts and documents review.

A complaint inspection was initiated on 8/6/21 and concluded on 8/11/21. The provider was contacted by telephone and a remote inspection was conducted. Information gathered during the inspection determined non-compliance with applicable standard or law and a violation notice was issued to the program.

Description: Based on interviews and a review of documentation it was determined that an area in the family day home was not maintained in a safe condition.
Evidence:
Interviews revealed that on 2/7/19 at approximately 8 am, child A, 2 years of age was playing and fell into the glass door of the gas fireplace. The gas fireplace was on and the glass door was hot and caused a first degree burn on the child's hand.

Plan of Correction: After the incident we immediately stopped using the fireplace during daycare hours and we also bought a cover for the fireplace.

Description: Based on interviews and a review of documentation it was determined that an unvented gas fireplace was used when children were in care. Unvented fireplaces shall not be used when children are in care.
Evidence:
Interviews revealed that on 2/7/19 at approximately 8 am, child A, 2 years of age was playing and fell into the glass door of the gas fireplace. The gas fireplace was on and the glass door was very hot. The provider took the child to a local medical center, the child was diagnosed with a first degree burn on her hand. A topical antibiotic skin ointment was prescribe. Interviews revealed that the gas fireplace was unvented.

Plan of Correction: After the incident we immediately stopped using the fireplace during daycare hours and we also bought a cover for the fireplace.
