Technical Assistance:

The Virginia Department of Education (VDOE) assumed oversight of child care licensing and monitoring on July 1, 2021. All associated regulations and codes have transferred over from the Virginia Department of Social Services. The violations listed below reflect the new VDOE codes and standards. A Crosswalk document has been given to the provider to reflect new new standard numbers. The application for renewal cannot be submitted until the CPS search results are reviewed.

Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program. A renewal inspection was initiated on 09-15-2021 and concluded on 09-15-2021. The provider was contacted by telephone and a virtual inspection was conducted. There was 1 children present, 2 years old, with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 1 child record and 4 staff/household records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #:	22VAC40-111-750-A
Description:	Based on a review of records, an authorization for topical skin products was not renewed annually. Evidence: The diaper ointment authorization for Child #1 that is available for review is dated 2019.
Plan of Correction:	I will have a new form signed by the parent.
Standard #:	22VAC40-111-120-A
Description:	Based on a review of records, one staff record does not contain all required information. Evidence: The record for Staff A, does not include documentation of an emergency contact, education and experience, 2 references.

Description:	Based on a record review, one adult record is incomplete. Evidence: Staff A's record does not contain documentation of orientation.

Description:	Based on a review of records, one staff record does not contain documentation of a completed CPS search. Evidence: The CPS search results for Staff B could not be located during the inspection.
Plan of Correction:	I have sent off a new search form, but the results have not been sent back yet. I will call OBI to see where the results are.

Description:	Based on a review of records, one staff record did not contain a completed Sworn Disclosure Statement (SDS) prior to becoming a resident of the home. Evidence: Staff A did not have a completed SDS on file. They became a resident "more than 30 days ago". No specific date given.
Plan of Correction:	Sworn Statement was completed during the inspection.
Standard #:	22VAC40-191-60-C-2
Description:	Based on a review of records, one staff record does not include documentation of a completed Central Registry Search (CPS) within 30 days of becoming a resident. Evidence: Staff A became a resident "more than 30 days ago" (no specific date given). The search results from the Central Registry are not available for review.
Plan of Correction:	The forms were mailed off, but have not come back yet. The forms were returned for being incomplete and sent off again. They have not been returned yet. I will call OBI to see where the results are.
