Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program. A monitoring inspection was initiated on September 30, 2021 and concluded on September 30, 2021. The provider was contacted by telephone and a virtual inspection was conducted. There were four children present, ranging in ages from nine months to four years, with one staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of two child records, one caregivers record and one household members record were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #:	22VAC40-111-60-B
Description:	Based on review of two children's records, the provider did not obtain all required information for each child's record. Evidence: 1. The record of child #1, did not contain the Insurance policy number. 2. The record of child #2, did not contain the Insurance policy number and the name, address and phone number of a second emergency contact.
Plan of Correction:	Per the provider: I will contact the parents today and have them complete the missing information in the child's record.
Standard #:	22VAC40-111-90-A
Description:	Based on review of children's records and interview, the provider did not obtain documentation that the child has been adequately immunized before the child attended the family day home. Evidence: 1. The record of child #1, enrolled on 09/07/2021, did not contain documentation of the child's immunization record. 2. The provider stated she had not obtained the immunization record for child #1.
Plan of Correction:	Per the provider: The family has a scheduled appointment next week with the child's physician. I will contact the parent today and have them provide documentation of the most recent immunizations for the record until they can be updated next week.
