Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and a virtual tour of the program. A monitoring inspection was conducted on 9/16/2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 5 children present with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 3 staff and household member records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #:	22VAC40-111-800-B
Description:	Based on record review, the provider did not review or update the emergency preparedness plan at least annually. Evidence: The emergency plan was last reviewed on 4/20/2018.
Plan of Correction:	I fixed it in the same day during the inspection.
Standard #:	22VAC40-111-60-B
Description:	Based on record review, two of two children's records did not contain all required information. Evidence: 1. Child 1's record did not contain the street address for emergency contacts. 2. Child 2's record did not contain the names, addresses, and phone numbers of two persons to contact in the event a parent cannot be reached.
Plan of Correction:	I talked to the parents and fixed it on Friday.
Standard #:	22VAC40-111-170-A
Description:	Based on record review, the provider did not obtain at the time of hire and prior to coming into contact with children a current Report of Tuberculosis Screening form published by the Virginia Department of Health or a form consistent with it documenting the absence of tuberculosis in a communicable form. Evidence: Staff 1 did not have documentation of a negative Tuberculosis screening on the date of inspection.
Plan of Correction:	I made an appointment for tomorrow to fix the issue.

Description:	Based on record review, the provided id not obtain for each caregiver and adult household member a current Report of Tuberculosis Screening form every two years from the date of the first screening or more frequently as recommended by a physician or the local health department. Evidence: 1. Adult 2's most current screening was conducted in November 2018. 2. Adult 3's most current screening was conducted in November 2018.
Plan of Correction:	My husband did his TB test last Friday.
