This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and a virtual tour of the program (if applicable). A (monitoring/renewal/complaint) inspection was initiated on (date) and concluded on (date). The director/provider was contacted by telephone and a virtual inspection was conducted. There were ___ children present with ___ staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. Description:	Based on record review and statement, the provider did not obtain for each caregiver and adult household member a current Report of Tuberculosis Screening form every two years from the date of the first screening or more frequently as recommended by a physician or the local health department. Evidence: 1. Adult 1's most recent Tuberculosis screening was conducted in 2018. 2. Adult 3's most recent Tuberculosis screening was conducted in January of 2018.
