Technical Assistance:

Standard 840. A and B - Injury Records was reviewed. A copy of the model form for recording children's injuries was emailed to the provider.

Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program. A complaint inspection was conducted on 09/09/2021 and 09/15/2021 in response to an allegation of inadequate supervision and that the provider was not maintaining a record of children's injuries. Based on the information gathered the allegation regarding inadequate supervision is found to be not valid. The provider failed to maintain written documentation of children's injuries therefore this allegation is found to be valid. The violation is listed on the violation notice issued to the provider on 09/15/2021.

Description:	Based on interview, the provider failed to record in the child's record an injury sustained by a child while at the family day home that requires first aid. Evidence: 1. The provider confirmed that she had not recorded multiple biting injuries sustained by a child while in care in August of 2021. a. The provider stated that she placed a cool wet towel on the area that had been bitten.
Plan of Correction:	A record was not kept because the bite didn't require first aid. There were never bites that caused swelling, redness. I wasn't aware a log was required. I have never had a child injured in my care.
