Comments:
A renewal inspection was initiated on 9/29/21 and concluded on 10/1/21. An inspection was conducted on site on 9/29/21 to observe the care of children and the physical plant. The provider was contacted by email on 9/30/21 to provide documentation to complete the inspection. There were five children present, ranging in ages from 15 months to three years, with the provider supervising. The inspector reviewed compliance in the areas of administration, physical plant, supervision, programming, special care and emergencies and nutrition. A total of two children's records and the provider's and household member's records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-111-750-B
Description: Based on observation, the provider failed to keep topical skin products inaccessible to children.

Evidence:

A tube of Desitin diaper ointment was stored on an open shelf under the changing table. The changing table is located in the main room used by children.

Plan of Correction: The diaper ointment will be moved to a location inaccessible to children. In the future the provider will ensure all topical skin products are kept in a location inaccessible to children.

Description: Based on observation, the provider failed to store poisonous substances in areas inaccessible to children.

Evidence:

A spray bottle labeled bleach water was stored on an open shelf under the changing table. The changing table is located in the main room used by children.

Plan of Correction: The spray bottle of bleach water will be moved to a location inaccessible to children. In the future the provider will ensure all poisonous substances are inaccessible to children.

Standard #: 22VAC40-111-690-D
Description: Based on observation, the provider failed to clean and sanitize the diapering surface after diapering.

Evidence:

1. After diapering the provider was observed spraying the diapering surface with bleach water and immediately wiping the surface down. The diapering surface was not cleaned.
2. The provider stated she thought bleach cleaned and sanitized the surface.

Plan of Correction: The provider will make a spray bottle of soapy water and will clean the diaper surface with the soapy water before spraying the surface with the bleach water. Once the surface is sprayed with the bleach water she will let the bleach water sit on the surface while she washes the hands of herself and the child's before wiping the surface off.

Standard #: 63.2-1721.1-B-2
Description: Based on record review and interview, the provider failed to ensure a household member submitted fingerprint results within 30 days after turning 18.

Evidence:

1. Per documentation household member 1 turned 18 in June.
2. The provider verified household member 1 has not submitted fingerprint results.

Plan of Correction: An appointment for 10/2/21 has been set for household member 1 to get fingerprints completed. Once the results are received they will be forwarded to the licensing inspector.
