63.2 Licensure and Registration Procedures

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was initiated on September 28, 2021 and concluded on September 29, 2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 9 children present, ranging in ages from 7 months to 3 years, with 2 caregivers supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records, 1 household member record and 2 caregiver records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Violations:
Standard #: 22VAC40-111-180-A
Description: Based on a review of caregiver records and interview, the provider did not ensure to obtain a tuberculosis screening for each caregiver every two years from the date of the first screening or more frequently as recommended by a physician or the local health department.
Evidence: 1. The record of Caregiver #1 contained documentation of a chest x-ray dated 4/2/19.
2. The record of Caregiver #2 contained documentation of a tuberculosis test dated 4/22/19.
3. The provider acknowledged they did not have updated tuberculosis screenings.

Description: Based on a review of records and interview, the provider failed to obtain central registry search results every five years for all employees.
Evidence: 1. The record of Caregiver #1 did not contain documentation of a repeat central registry check. The most recent central registry result in the record is dated 8/1/16.
2. The record of Caregiver #2 did not contain documentation of a repeat central registry check. The most recent central registry result in the record is dated 8/4/16.
3. The provider acknowledged the central registry checks were expired.
