Technical Assistance:
Technical assistance was offered regrading the following: children's records, personnel and household member records, zoning and capacity, outdoor deck used for children, infant bottles/labeling food from home, diapering space and cleaning procedures, and licensing/license renewal procedures at the end of the conditional period.

Comments:
An announced initial inspection was conducted on 9/30/21 from 9:50 am to 11:15 am. There was 1 child (4 points) present, with 1 caregiver. A walk through of the family day home was completed, including the outdoor deck space. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. The personnel and household member records were reviewed prior to the in-person inspection. One child's record was reviewed. Emergency supplies, first aid kit, and 2 cribs were assessed. The provider has stated that all attending children will bring food from home.

Violations:
Standard #: 22VAC40-111-770
Description: Based on observation, the family day home did not have a working battery operated flashlight, a working portable battery operated weather band radio, and extra batteries shall be kept in a designated area and be available to caregivers at all times.
Evidence:
1. The provider had a battery operated flashlight with no batteries. The flashlight did not work during the inspection
2. A weather band radio was not available.

Plan of Correction: Batteries have been purchased and an order for a weather band radio is on the way.

Standard #: 22VAC40-111-790
Description: Based on observation, emergency numbers were not posted.
Evidence: A 911 or local dial number for police, fire, and emergency medical responders was not posted anywhere in the family day home.

Plan of Correction: The numbers have been written and stored at the entrance of the home day care.

Standard #: 22VAC40-111-240-A
Description: Based on observations, areas inside and outside the family day home were not maintained in a safe condition.
Evidence:
1. Peeling paint was found on the wall at the bottom of the stairs leading into the daycare area.
2. A green rug on the outdoor deck and two other plastic mats in front of the rug were not evenly laid out, posing a trip hazard.

Plan of Correction: Paint has been painted over as of 10/5. The green runs are now evenly laid out.

Standard #: 22VAC40-111-320-A
Description: Based on observation, small electrical appliances were plugged in while not in use.
Evidence: Two small electrical appliances in the kitchen were left plugged in while not in use.

Plan of Correction: Electric appliances will always be unplugged and stored.

Standard #: 22VAC40-111-330-A
Description: Based on observation and staff interview, a landline telephone, excluding a cordless or cell phone, was not available, operable, and accessible during the family day home's hours of operation.
Evidence: The provider stated that the family day home did not have a landline installed and functional.

Plan of Correction: A landline will be installed.

Standard #: 22VAC40-111-430-B
Description: Based on observation, accordion expansion gates and pressure mounted gates were used as protective barriers at stair openings.
Evidence:
1. An accordion gate was installed at the opening by the stairs that led to the upstairs from the daycare space in the basement of the family day home.
2. A pressure mounted gate was used as a barrier on the deck at the stairs that led to the lower level of the backyard. The gate was not fully installed and loose on the right side.

Plan of Correction: A new gate has been purchased for both the staircase and the outside deck stairs. Both will be fully installed.
