Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews) and a virtual tour of the program. A monitoring inspection was initiated on 10/4/2021 and concluded on 10/4/2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 5 children present, ranging in ages from 20 months to 2 years, with 1 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 4 staff/ household member records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards and a violation was documented on the violation notice issued to the program.

Description:	Based on a review of caregiver and household member records, the provider has not obtained a current TB screening every two years from the date of the last screening. Evidence: Caregiver #1 TB screening is dated 3/18/2019, Household member #1 TB screening is dated 8/11/2019, Household member #2 TB screening is dated 8/23/2019.
Plan of Correction:	The caregiver is scheduled for 10/6/2021 to update TB screening. The household members will get update when they return from out of town.
