Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A renewal inspection was initiated on October 6, 2021 and concluded on October 6, 2021. The provider was contacted by telephone and a virtual inspection was conducted. There were five children present, ranging in ages from three months to six years, with two caregivers supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of two child records and two caregiver records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-111-90-A
Description: Based on review of children's records, the provider did not obtain documentation that each child had been adequately immunized before the child attended the family day home.

Evidence:

1. The record of child #1, enrolled 08/30/2021, had documentation of immunizations dated 09/03/2021 exceeding the first day of attendance.
2. The records of child #2, enrolled 10/04/2021, did not contain documentation of immunizations.

Plan of Correction: Per the provider: I thought I had collected all the required information. I contacted the parent and she had the documents and will provide them to me today. Going forward, I will make sure parents provide the immunizations by their first day.

Standard #: 22VAC40-111-180-A
Description: Based on review of caregiver records and interview, the provider did not obtain a current Report of Tuberculosis Screening form for each caregiver and adult household member every two years from the date of the first screening.

Evidence:

1. The most recent tuberculosis screening for caregiver #1 was dated 09/05/2019, exceeding the requirement to renew every two years from the date of the first screening.
2. Caregiver #1 stated she had not obtained an updated tuberculosis screening.

Plan of Correction: Per the provider: I have scheduled an appointment to update the tuberculosis screening for next week.

Standard #: 22VAC40-111-280
Description: Based on observation and interview, the provider did not ensure potentially poisonous substances, materials and supplies were stored away from food in areas inaccessible to children.

Evidence:

During the virtual tour of the home, a bottle of mouthwash was observed on the counter of the bathroom sink. The bathroom is used by children in care.

Plan of Correction: The provider removed the bottle of mouthwash and place it in the locked bedroom.

Standard #: 22VAC40-111-550
Description: Based on interview, the provider did not ensure where used, a play pen shall not be the designated sleeping area.

Evidence:

1. The provider stated that child #2, 3 months in age, slept in the pack-n-play daily that was provided by the parent.
2. During the time of the inspection, the unassembled crib was being stored in the shed.

Plan of Correction: Per the provider: I have a crib that has not been open yet packed in the shed. I will take it out and set it up today to use for the infants sleeping time.
