Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program. A monitoring inspection was initiated and concluded on October 14, 2021. The provider was contacted by telephone and a virtual inspection was conducted. There were six children(18pts) present, ranging in ages from 10 months to 2 years old, with two staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of two child records and two staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #:	22VAC40-111-70-A
Description:	Based on record review, children's records did not contain the required information. Evidence: 1. Child #1's record(Start date-08/02/2021) did not contain documentation of the Provisions of the Emergency Plan.
Plan of Correction:	The provider will give the parents a copy of the Provisions of the Emergency Plan and have the parents to sign it.
Standard #:	22VAC40-111-140-B-3
Description:	Based on record review and interview, the caregiver did not obtain current certification in cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care. Evidence: 1. Staff #1 did not have documentation of current certification in CPR. The CPR expired on 03/16/2021. 3. Staff #1 stated that the certification had expired and is actively looking for classes.
Plan of Correction:	The provider has been looking for CPR classes to take. The provider will take a CPR class and renew the certification
Standard #:	22VAC40-111-140-B-4
Description:	Based on record review, the caregiver did not obtain current certification in first aid. Evidence: 1. Staff #1 did not have documentation of current certification in first aid. The first aid certificate expired on 03/15/2021.
Plan of Correction:	The caregiver will recertify in first aid as soon as a class is available.
Standard #:	22VAC40-111-150-D-1
Description:	Based on record review, staff did not obtain current certification in cardiopulmonary resuscitation (CPR). Evidence: 1. Staff #2 did not have documentation of current certification in CPR. The CPR certification expired on 03/16/2021.
Plan of Correction:	Staff will take a CPR class as soon as possible.

Description:	Based on record review, staff did not obtain current certification in first aid. Evidence: 1. Staff #2 did not have documentation of current certification in first aid. The first aid certificate expired on 03/15/2021.
Plan of Correction:	Staff will take a class for fist aid and recertify.
Standard #:	22VAC40-111-180-A
Description:	Based on record review and interview, the provider did not obtain for each caregiver a current Report of Tuberculosis Screening form, in accordance with the requirements in 22 VAC 40-111-170, every two years from the date of the first screening. Evidence: 1. Staff #1 did not have documentation of a subsequent tuberculosis screening in the record. The last tuberculosis test was dated 03/14/2019. 2. Staff #1 stated that a current tuberculosis screening had not been completed.
Plan of Correction:	The staff member will get a tuberculosis screening from the doctor.
