This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program. A renewal inspection was initiated and concluded on October 19, 2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 11 children with 3 caregivers supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care, and emergencies and nutrition. A total of 3 children's records and 3 caregiver records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards were documented on the violation notice issued to the program. Standard #: 22VAC40-111-180-A Description: Based on a review of caregiver and household member's records, the provider failed to obtain for each caregiver and adult household member a current report of Tuberculosis Screening form in accordance with the requirements every two years from the date of the first screening or more frequently as recommended by a physician or the local health department.
Evidence:
Caregiver #1,#2 and #3 do not have documentation of a current TB screening. All caregiver's TB screening expired in September 2021. Household member #1 does not have documentation of a current TB screening.Plan of Correction: All caregiver's and household members will obtain current TB screening.
