Technical Assistance:
Today we discussed / clarified the requirement that no medication be administered to children in care unless or until the provider has obtained the required / approved medication administration training. This includes both prescription and over the counter medications.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection during the 6 month conditional licensure period was initiated on 9/20/2021 and concluded on 10/15/2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 2 children present, ranging in ages from 14 months to 3 years of age with the licensee supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 3 staff / household member records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-111-60-B
Description: Based on a review of records and interview, the licensee was not able to demonstrate that each enrolled child had a complete record which contained all of the required documentation.
Evidence:
The records for child #2 and child #3 did not contain the name and policy number of the child's medical insurance. Child #2's record had no information written pertaining to the child's medical insurance. The record for child #3 contained the insurance provider's name but no policy number.
The record for child #3 did not contain the name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached. Only one emergency contact person's information was documented in the record for child #3.
The licensee confirmed that she did not have the aforementioned documentation for child #2 and child #3.

Plan of Correction: The licensee stated that she would speak to the parents of child #2 and child #3 and obtain the missing documentation within 10 days.

Standard #: 22VAC40-111-70-A
Description: Based on a review of records and interview, the licensee did not ensure that before each child's first day of attendance, the child's parents are provided in writing information regarding whether or not there is liability insurance of at least $100,000 per occurrence and $300,000 aggregate in force on the family day home operation as required by ? 63.2-1809.1 of the Code of Virginia.
Evidence:
The record for child #3 contained a form with the required information. However, there was no signature on the form indicating that the parent had received the information.

Plan of Correction: The licensee stated that she would review the form with the parent and have the parent sign acknowledgement of receipt within 10 days.
