Technical Assistance was offered regrading: Diapering procedures, background checks,

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A renewal inspection was initiated and concluded on 10/13/21. The provider was contacted by telephone and a virtual inspection was conducted. There were 2 children (8 points) present, with 2 caregivers supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medications, special care and emergencies and nutrition. The lunch menu included chicken nuggets, strawberries, peas, veggie chips and milk. Diapering was observed. The outdoor play area was observed. A total of 2 children's records, 2 caregivers and 2 household member records were reviewed.

Standard #: 22VAC40-111-60-B
Description: Based on records review, children's records were not complete.
Evidence: Child # 1's record was missing the following information: a. At least one parent's complete work information; b. Second emergency contact information; c. medical insurance policy information including policy number.

Plan of Correction: Child # 1 no longer attends Martha's daycare. Child # 1's last day of attendance was October 15, 2021.

Standard #: 22VAC40-111-320-G
Description: Based on records review, the provider did not get an annual inspection done for a wood burning stove and associated chimney by a knowledgeable inspector to verify that the devices are properly installed, maintained, and cleaned as needed.
Evidence: The most recent inspection for the wood burning stove in the family day home's kitchen was dated 1/30/19.

Plan of Correction: The annual inspection was conducted on October 20, 2021. A scan of the annual inspection was sent by e-mail on October 21, 2021.

Standard #: 22VAC40-111-460-C
Description: Based on staff interview and record review, rabies vaccinations for the family pet were not updated.
Evidence: The provider stated that vaccination records for the pet cat were not updated. The most recent rabies vaccination for the cat was dated 6/15/16.

Plan of Correction: The household member who owns the cat will acquire the vaccination records on October 27, 2021 at 9:30 am.

Standard #: 63.2-1721.1-A
Description: Based on records review, the provider did not obtain repeat background checks for adult household members every 5 years.
Evidence:
1. The sworn disclosure statements (SDS) for Household Member # 1 and Household Member # 2 were dated 4/27/16.
2. The SDS for both household members were renewed on 10/14/21.

Plan of Correction: All background checks will be renewed on time.
