A modification and monitoring inspection was conducted on 10/19/21. The provider has moved to a single family home. The basement level, with full access to the back yard, will be used for care. The basement is still being renovated. The area set up for use upstairs, is bright and full of lots of supplies for the children to explore. Observed the children playing and interacting with the provider and assistant. 4 children in care with two caregivers. A total of 10 points. Records review conducted after the physical inspection.
