Technical Assistance:
Discussion with the Provider that her Business Entity is a LLC. Therefore, in order for her application to be considered complete a copy of the Articles of Organization for the LLC will need to be kept on file. The Provider stated that she will send.

Comments:
A monitoring inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and interview.

This inspection was initiated and concluded on 10/22/2021. The Provider was contacted by telephone and the inspection was conducted. There were 4 children present (13 points), ranging from infant to preschool age, with 2 caregivers supervising. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; medication (no medication); special care and emergencies; and nutrition (meals and snacks provided by child's parent). A total of 6 children records, 4 household member records, and 2 staff records were reviewed.

Information gathered during the inspection determined that there were two (2) non-compliances with applicable standards. These violations were documented on the Violation Notice issued to the program.

Violations:
Standard #: 22VAC40-111-90-C
Description: Based on review, 2 of 6 children files determined that the Family Day Home (FDH) did not obtain documentation of additional tuberculosis (TB) immunizations for children (who were not exempt from the immunization requirements according to subsection B of this section) once every six months for children under the age of two years.

Evidence -
1. On the date of inspection (10/22/2021), the following children had TB documentation older than 6 months: Child #4 (TB documentation dated - 04/08/2021), and Child #5 (TB documentation dated - 03/30/2021).
2. This finding was confirmed on the date of inspection by the Provider.

Plan of Correction: Updated immunization documentation shall be obtained for the children listed above and placed in the applicable child's file.

Standard #: 22VAC40-111-210-A
Description: Based on review, 2 of 2 staff files determined that both caregivers did not obtain a minimum of 16 hours of training annually in areas relevant to their job responsibilities.

Evidence -
1. On the date of inspection (10/22/2021), the records for Staff #2 indicated that 0 hours of annual training were obtained within the past year.
2. On the date of inspection, the records for Staff #1 indicated that 5 hours of annual training were obtained within the past year.

Plan of Correction: Due to the pandemic, training was difficult to schedule and obtain. At least 16 hours of annual training shall be obtained for each caregiver for the coming training year. Some training has already been scheduled.
