Technical Assistance:
The Virginia Department of Education (VDOE) assumed oversight of child care licensing and monitoring on July 1, 2021. All associated regulations and codes have transferred over from the Virginia Department of Social Services. The violations listed below reflect the new VDOE codes and standards. A code crosswalk document was provided to show the new regulation set numbers.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program. A monitoring inspection was initiated on 10/19/2021 and concluded on 10/21/2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 2 children present, ranging in ages from 4 months to 4 years, with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 3 staff/household member records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 22VAC40-111-60-B
Description: Based on a review of records, one child record does not contain all required documentation.
Evidence;
1 of 2 records reviewed did not contain documentation of the insurance policy number. (Child 1)

Plan of Correction: The parent will be asked to provide the information.

Standard #: 22VAC40-111-70-A
Description: Based on a review of records, one required policy has not been updated, and provided in writing to parents as needed.
Evidence:
The decision to administer medication policy needed to be updated in January 2021 when the Medication Administration Training (MAT) training certification expired (1/25/21). Medication cannot be administered until MAT certification is current. (Children #1, 2)

Plan of Correction: I will update the medication policy in all child records.

Standard #: 22VAC40-111-140-B-3
Description: Based on a review of documentation, Cardiopulmonary resuscitation (CPR) certification is not current for any caregiver.
Evidence:
The CPR certification for both caregivers expired on 3/16/2021. (Caregiver A, B)

Description: Based on a review of records, caregiver and household records do not contain documentation of repeated Central Registry Searches (CPS) in the allowable 5 year time frame.
Evidence:
3 of 3 records reviewed contained documentation of CPS searches dated 10/11/12016 (expired).

Plan of Correction: New search forms will be submitted as soon as possible.
