Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was initiated on 10/26/2021 and concluded on 11/1/2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 4 children present, ranging in ages from 2 to 3 years old, with 1 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 2 provider / household records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-111-100-A
Description: Based on a review of records and interview, the provider was unable to demonstrate that she had obtained documentation of a physical examination by or under the direction of a physician prior to each child's attendance or within 30 days after the first day of attendance.

Evidence:
Two children's records were reviewed during the inspection.
The records for child #1 (enrolled 9/20/2021) and child #2 (enrolled 4/22/2021) did not contain documentation of a physical examination. The provider stated that she did not have this documentation.

Plan of Correction: The provider will request that parents provide documentation of the results of a physical for child #1 and child #2 within 10 days.

Standard #: 22VAC40-111-60-B
Description: Based on a review of documentation and interview, the licensee did not ensure that each child's record contained all of the required information and documentation.
Evidence:
Two children's records were reviewed during the inspection.
The record for child #2 did not contain the policy number of the child's medical insurance.
The records for child #1 and child #2 did not contain written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately.
The provider stated that she did not have this documentation.

Plan of Correction: The provider sought assistance in obtaining model forms for capturing the required documentation and will have parents complete the missing information within 10 days.

Standard #: 22VAC40-111-70-A
Description: Based on a review of documentation and interview, the licensee did not ensure that each child's record contained all of the required information and documentation.
Evidence:
Two children's records were reviewed during the inspection.
The records for child #1 and child #2 did not contain documentation that the family was provided with Provisions of the family day home's emergency preparedness and response plan.
The provider stated that she did not have this documentation.

Plan of Correction: The provider sought assistance in obtaining a model form for sharing these provisions and will complete this information and provide to the parents of enrolled children within 10 days.
