Technical Assistance:
When the results for the central registry are received, send them to your inspector. If the results are not received within 30 days you will need to follow up on the status of the results.
Staff cannot work until an eligible fingerprint is on record. Staff will contact Fieldprint to set up an appointment using my provider code, to have their fingerprint completed. Eligible fingerprint letter will be forwarded to your inspector.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews (if applicable) and a virtual tour of the program (if applicable).

A renewal inspection was initiated on November 1, 2021 and concluded on November 5, 2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 6 children present, ranging in ages from 1 month to 5 years with 1 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 2 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-111-180-A
Description: Based on review of provider's record, there is no documentation of a current report of tuberculosis screen form.
Evidence:
The record for provider #1 contains expired documentation of a report of tuberculosis screen form.

Plan of Correction: I have an appointment on November 17 to get my tuberculosis form completed.

Standard #: 22VAC40-111-210-A
Description: Based on review of provider's record there is not documentation of annual training hours.
Evidence
The record for provider #1 does not have any documentation of completing any annual trainings.

Plan of Correction: I took some classes but didn't receive any certificates at the end. I will make sure to get and print a certificate for all the trainings I take.

Description: Based on review of providers record, a central registry check was not completed before five years since the date of the last central registry check.
Evidence
