Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A renewal inspection was initiated and concluded on 11/15/2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 6 children present, ranging in ages from 15 months to 3 years, with 1 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 2 staff / household member records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-111-100-A
Description: Based on a review of records and interview, the provider did not obtain results of a physical examination within 30 days of the child attending the family day home.
Evidence:
Two children's records were reviewed.
The record for child #2, whose first date of attendance was 9/9/2021, did not contain documentation of the results of a physical examination.
When asked, the provider stated that she does not have results of a physical examination for child #2.

Plan of Correction: The provider has spoken to child #2's parent requesting this documentation and the parent told her that she has an appointment scheduled for this week to obtain an updated physical exam which she will give to the provider by the end of the week.
In the future, results of a physical examination will be obtained within 30 days of a child attending the family day home.

Standard #: 22VAC40-111-90-A
Description: Based on a review of records and interview, the provider did not ensure that before a child may attend the family day home, documentation is obtained demonstrating that that the child has been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations.

Evidence:
Two children's records were reviewed.
The record for child #2 whose first date of attendance was 9/9/2021, did not contain documentation regarding any immunizations received.
When asked, the provider stated that she does not have an immunization record for child #2.

Plan of Correction: The provider has spoken to child #2's parent requesting this documentation and the parent told her that she has an appointment scheduled for this week to obtain an updated record of immunizations which she will give to the provider by the end of the week.
In the future, documentation of immunizations will be obtained prior to any child attending the family day home.
