Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was initiated on October 26, 2021 and concluded on November 4, 2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 5 children present, ranging in ages from 10 months to age 3, with 1 provider supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records, 1 provider record and 1 household record were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-111-730-D
Description: Based on virtual observation and provider interviews, the provider did not ensure medication, except for those prescriptions designated otherwise by a written physician's order, including refrigerated medication and medications for caregivers and household members, shall be kept in a locked place using a safe locking method that prevents access by children.

Evidence: A container of non-prescription topical medication (cortisone cream) and a container of non-prescription pain reliever medication were observed in an unlocked plastic bin in the childcare room.

Plan of Correction: Lock up all prescription and nonprescription medication.

Standard #: 22VAC40-111-750-A
Description: Based on virtual observation and provider interviews, the provider did not ensure that when topical skin products such as sunscreen, diaper ointment and lotion, oral teething medicine, and insect repellent are used, the product shall be in the original container and labeled with the child's name.

Evidence: A container of diaper cream was observed and it was not labeled with a child's name. The provider confirmed it belong to the child #3 who was in care.

Plan of Correction: Put child's name on container and put into original container.

Standard #: 22VAC40-111-760-A-1
Description: Based on virtual observation and provider interviews, the provider did not ensure the first aid kit that contained all minimum items.

Evidence: The first aid kit supplies were inadequate as it did not include gloves, tweezers and first aid tape The provider confirmed the missing required items.

Plan of Correction: Purchase all missing items and replace in container.

Standard #: 22VAC40-111-140-B-3
Description: Based on review of provider interviews and documents, the provider did not ensure to obtain current certification in cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care, from the American Red Cross, American Heart Association, American Safety and Health Institute, or the National Safety Council, or current CPR certification issued within the past two years by a community college, a hospital, a rescue squad, or a fire department.

Evidence: The provider's certification card available for review was dated with an expiration of 7-20-2021.

Plan of Correction: Renew my CPR class as soon as possible.

Standard #: 22VAC40-111-140-B-4
Description: Based on review of provider interviews and documents, the provider did not ensure to obtain current certification in first aid from the American Red Cross, American Heart Association, American Safety and Health Institute, or the National Safety Council, or current first aid certification issued within the past three years by a community college, a hospital, a rescue squad, or a fire department.

Evidence: The provider's certification card available for review was dated with an expiration of 7-20-2021.

Plan of Correction: Renew my First Aid as soon as possible.

Standard #: 22VAC40-111-240-A
Description: Based on virtual observation and provider interviews, the provider was unable to demonstrate that all areas and furnishings of the family day home, inside and outside, shall be are maintained in a clean, safe, and operable condition.

Evidence: The provider was unable to provide documentation to demonstrate the crib observed during the virtual inspection met the compliance requirements for the new federal crib standards. The provider confirmed the crib was designated for child #3 in care.

Plan of Correction: Purchase a new crib with paperwork.

Standard #: 22VAC40-111-690-D
Description: Based on virtual observation and provider interviews, the provider did not ensure the diapering surface was non-absorbent and washable.

Evidence: The diaper pad observed was inadequate as it had a cloth cover and was not non-absorbent and washable.

Plan of Correction: Take the cover off and use and use washable only.
