Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was initiated on November 18, 2021 and concluded on November 19, 2021. The provider was contacted by telephone and a virtual inspection was conducted. There were six children present with two caregivers supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of two child records, one caregiver and one household member records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-111-60-B
Description: Based on review, provider did not ensure 1 of 2 children's records has documentation of required information.

Evidence: The record for child #2 (start date October 14, 20219) did not have father's employment address and employment phone number.

Plan of Correction: Provider will request information from parent today.

Standard #: 63.2-1721.1-A
Description: Based on review, provider did not ensure required background checks are completed every five years.

Evidence: The record for caregiver #1 has a most recent Central Registry Check completed January 8, 2016. The record for household member #1 has a most recent Central Registry Check completed April 19, 2016. The Central Registry Checks were not updated every five years.

Plan of Correction: Both Central Registry Checks have been filled out. Signatures will be notarized and checks will be mailed within a week.
