Comments:
A renewal inspection was conducted on site today, November 12, 2021. The provider was present and partisipated in the renewal inspection. There were six family day home children and two resident children in care with two caregivers for a value of 19 points. Children ranged in ages from 14 months to four years. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of two child records, three caregiver records and one household member record were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-111-60-A
Description: Based on review of children's records, the provider did not obtain all required information for one child's record.

Evidence:

The record for child #1, did not contain the name, address and phone number of a second designated person to be contacted in case of an emergency if the parent cannot be reached.

Plan of Correction: Per the provider: I will request that the parent provide a second emergency contact for the record.

Standard #: 22VAC40-111-290
Description: Based on observation and interview, the provider did not ensure sharp kitchen utensils were inaccessible to children unless being used by the caregiver or with children under close supervision.

Evidence:

1. The provider stated sharp kitchen knives were being kept in the unlocked kitchen drawer.
2. The kitchen drawer contained sharp knives and were accessible to children in care.

Plan of Correction: Per the provider: I will install a child safety lock on the kitchen drawer.
