This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews (if applicable) and a virtual tour of the program (if applicable).
A monitoring inspection was initiated on Monday, November 22, 2021 and concluded on Tuesday, November 23, 2021 .The provider was contacted by telephone and a virtual inspection was conducted. There were 7 children present and 2 caregivers present supervising. The inspector reviewed compliance in the areas of administration, personnel, household member, physical health, physical equipment and environment, care of children, preventing the spread of disease, emergencies, and background checks. A total of 3 child records and 2 caregiver/household member records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Evidence: A review of the provider's two pets vaccination was documented due dates on November 4, 2021.Plan of Correction: The provider has scheduled appointments on December 12, 2021 for both pets.
