Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews, and a virtual tour of the program.

A monitoring inspection was initiated on November 22, 2021 and concluded on November 24,2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 11 children present, ranging in ages from 16 months to 3 years, with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 2 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Please call with questions 703-479-4679

Violations:
Standard #: 22VAC40-111-30-B
Description: Based on interview, record review and observation, it was determined the provider did not ensure compliance with the home's policies that have been disclosed to the parents.
Evidence:
1. The provider is not MAT or EMAT certified and has a written policy that she will not administer medication.
2. The provider stated she has a child enrolled with a serious food allergy and that there is prescription medication at the home in the event the child has an allergic reaction.
3. The child's file was reviewed and the enrollment form states the food allergies and the plan of action in an emergency states "epipen'. The form on whether the Provider will or will not administer medication has not been completed or signed by the parent to acknowledge medication will not be given.
3. The medication present does not include a long term authorization form to store the medication.

Plan of Correction: I will have parent sign the medication acknowledgment form and I will sign up for EMAT course.
