This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review and interviews.
A monitoring inspection was initiated on 11/15/2021 and concluded on 11/18/2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 4 children present, ranging in ages from 4 years to 5 years, with 1 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 1 child record and 1 staff record were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 22VAC40-111-800-B Description: Based upon review of the Emergency Preparedness Plan (EPP) and provider interview, the provider has not ensured that the emergency preparedness plan is reviewed annually and the review is documented.
Evidence:
1)The most recent review of the Emergency Preparedness Plan documented by the provider was dated 09/15/2019. 2) The provider stated that they have not reviewed the EPP since.Plan of Correction: Will review the emergency plan and document it.
Standard #: 22VAC40-111-60-B Description: Based on record review, children's records did not contain all required information.
Evidence:
1) The record for Child #1 did not contain the name, address and telephone number of a second designated person to contact in case of an emergency if the parent cannot be reached.Plan of Correction: Will contact the parents to obtain the required information.
