Technical Assistance:
Technical Assistance was provided regarding the following:
1. Carbon Monoxide Detectors are required for all licensed family days homes that serve preschool age children operating in a building built before 2015.

Comments:
Today an unannounced renewal inspection was conducted with Nosheen Iftikhar. This visit took place in the morning between 9:55am-11:15am. There were four children present with the Provider and two assistants for a total of 14 points within the supervision guidelines. The children were observed playing with blocks, being read a story, and playing with various manipulative toys. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. A total of three child records and three staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-111-800-B
Description: Based on record review, the provider did not review the emergency plan at least annually and update the plan as needed. The provider did not document in writing each review and update to the emergency plan.

Evidence:
1. The Emergency plan was not reviewed and updated in writing. The last time that the Emergency Plan was reviewed and documented was 08/26/2020.

Plan of Correction: The provider reviewed and documented the Emergency Plan during the time of the inspection.

Standard #: 22VAC40-111-60-B
Description: Based on record review, children's records did not contain required information.
Evidence:
1. Child #2's record (Start date-08/30/2021) did not contain the medical insurance policy number.

Plan of Correction: The provider will get the medical insurance policy number from the parents and document the record.

Standard #: 22VAC40-111-180-A
Description: Based on record review, the provider did not obtain for each caregiver and adult household member a current Report of Tuberculosis Screening form, in accordance with the requirements in 22 VAC 40-111-170, every two years from the date of the first screening or more frequently as recommended by a physician or the local health department.
Evidence:
1. Staff #1's record did not contain documentation of subsequent tuberculosis screening. The last tuberculosis test was expired and dated 10/2/2019.

Plan of Correction: The provider will go to the minute clinic and have a tuberculosis screening completed.

Standard #: 22VAC40-111-530-F
Description: Based on observation, soft bedding was used under or around infants.
Evidence:
1. Child #2 (6 months old) was inside of a crib with a stuffed toy positioned under their head and a blanket was wrapped around their body.

Plan of Correction: The stuffed toy was removed from the crib and the blanket was removed during the time of the inspection.

Standard #: 22VAC40-111-680-A
Description: Based on observation, caregivers did not wash their hands with liquid soap and warm running water after contact with any body fluids.
Evidence:
1. Staff #1 wiped Child #4's nose with a tissue and did not wash their hands after contact.

Plan of Correction: Staff will be retrained and will wash their hands after wiping children's noses.
