Technical Assistance:
Technical assistance was offered regarding infant bottles.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A renewal inspection was initiated and concluded on 12/13/21. The provider was contacted by telephone and a virtual inspection was conducted. There were 9 children (25 points) with 3 staff supervising. The children were observed completing a writing exercise and playing with toys. There were no medications on site. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. A total of 3 child records and 3 staff records were reviewed. Emergency preparedness plan and supplies were also reviewed.

Violations:
Standard #: 8VAC20-800-80-A
Description: Based on record review, the provider did not obtain verification of the identity and age of the child within 7 days of enrollment.
Evidence:
1. Child # 1's first day of attendance was 6/7/21.
2. Child # 1's record indicated that the document for verification of identity and age was not viewed until 6/29/21.

Plan of Correction: The documentation will be viewed within 7 days.

Standard #: 8VAC20-800-180-A
Description: Based on records review, the provider did not obtain obtain for each adult household member a current Report of Tuberculosis (TB) Screening form every two years from the date of the first screening.
Evidence:
1. Household member # 1's TB screening expired on 7/25/21. The subsequent TB screening was dated 9/17/21.
2. Household member # 2's TB screening expired on 8/29/21. The subsequent TB screening was dated 9/1/21.

Plan of Correction: TB screenings will be obtained in the right time frame.
